Matrix Care Management is seeking a detail-oriented and team-focused Care Management Assistant/ Associate to join our Care Management Team. This individual will play a key role in supporting daily operations, assisting with contacting the clients, making follow up physicians and other medical appointments. Maintaining effective communication with Matrix Care Managers. Key Responsibilities • Assist with staffing medical and non-medical appointments for our clients.
