The Youth Transition Services Facilitator assists teens and young adults with disabilities, ages 14 to 22, to acquire knowledge, skills and supports necessary to prepare for a fulfilling, self-directed life after high school. Working as part BCIL’s Transition to Adulthood Program (TAP) and Pre-Employment Transition Services (Pre-ETS) Programs, the Youth Transition Services Facilitator will be responsible for working with students individually as well as in groups in school classrooms throughout our service area, in the BCIL office, or remotely, through online video meeting platforms. Services provided include information and referral, peer mentoring, advocacy, skills training, and other supportive services. This position requires consistent in-person presence in order to most effectively work with students, as well as to collaborate other team members and school educators. Manages group dynamics and ensures that environment and facilitation strategies are supportive to all learners. · Ensures timely and thorough data collection and reporting (WILD, Statistical reports and Pre-ETS Student Progress Notes and other reports, as required) · Serves as a role model for young adults with disabilities · Performs other related duties as assigned.
