For years, the difficult conversation about taking away car keys has typically followed a crisis: a fender bender, getting lost close to home, or a noticeable progression in an Alzheimer's diagnosis. New evidence suggests we may be waiting too long. Research from a collaborative team at Florida Atlantic University and the University of Central Florida indicates that measurable changes in routine driving performance can act as digital biomarkers, signaling a decline in mental sharpness well before major symptoms of dementia become apparent.
The three-year study, monitoring 450 older drivers in their own vehicles using integrated sensors and cameras, moved beyond simulations and subjective surveys into the realm of real-world behavior. It found that drivers with mild cognitive impairment (MCI) exhibited distinct, quantifiable patterns. These included less consistent control of the accelerator and brake, more frequent episodes of hard braking, a tendency to take shorter and more fragmented trips, and an overall reduction in average driving speed. Critically, it was the combination of these behaviors, rather than any single action, that most clearly distinguished the two groups.
"Everyday driving habits—captured passively through in-car sensors—may offer a powerful new way to detect subtle cognitive changes long before they become obvious," explains Dr. Ruth Tappen, the study's senior author and a professor at FAU's Christine E. Lynn College of Nursing. Her team's work points to a future where a vehicle itself could contribute to health monitoring. "We're in the business of letting people know what may be happening with them," she emphasizes, noting the goal is proactive awareness, not immediate revocation of licenses.
The research aligns with observed compensatory strategies. Many individuals in early stages of cognitive decline may self-regulate, often unconsciously. They might avoid unfamiliar routes, shun highway driving, or perpetually remain in the right-hand lane. "These attempts to compensate for accumulating deficits eventually become insufficient to allow the individual to continue to drive safely," Dr. Tappen's research concludes. The Alzheimer's Foundation notes that individuals with Alzheimer's are at a significantly higher risk for at-fault crashes.
Beyond the Lab: Family Dilemmas and Personal Vigilance
The human dimension of this research is fraught with emotion and complexity. Family members often become the first witnesses to concerning changes, yet initiating a conversation about driving can be one of the most challenging tasks they face.
"If you notice something is different when driving with mom and dad or a spouse, have those conversations and get a plan in place," advises Jennifer Braisted of the Alzheimer's Association. "I have seen many instances where the family wished they had done something sooner." Resistance, anger, and a sense of lost independence are common reactions from seniors, for whom driving is deeply intertwined with autonomy.
Lorraine Marchand, an 80-year-old participant in the FAU study, observes these subtle shifts in peers. "They don’t get lost... but they’re more cautious. They stick to the right-hand lane all the time and go really slow," she notes. Yet, she understands the profound impact of losing driving privileges, which can lead to social isolation and reduced access to essentials.
Priscilla Jean-Louis recounts the extreme measures she had to take with her mother, Vera Johnson, who continued driving despite clear danger. "She was driving in the wrong direction, stopping at green lights and running red lights," Jean-Louis says. "My daughters were scared to ride with Granny." She ultimately disabled the car battery to prevent her mother from driving—a drastic step that highlights the potential crisis point families hope to avoid through earlier detection.
A New Path Forward: Technology, Treatment, and Targeted Testing
An important nuance in the driving-and-dementia discussion is that a diagnosis does not automatically mean an immediate end to driving. Approximately one in three people diagnosed with dementia continues to drive, especially as new medications and therapies can slow disease progression. The decision must be individualized and regularly reassessed.
Christine Burger, 73, who experiences moments of confusion at familiar intersections, embodies this balanced approach. After receiving a diagnosis and beginning medication and cognitive therapy, she continues to drive with vigilance and a plan. "The minute the... doctor involved in my treatment says 'you can no longer drive,' I will not drive," she states. Her proactive stance—monitoring her own habits and following medical advice—is a model for managing risk while preserving independence.
Expert Insight: Leveraging Existing Car Tech for Self-Monitoring
A unique and actionable perspective emerging from this research is the potential to use widespread, existing vehicle technology for personal health awareness. Modern vehicles are increasingly equipped with advanced driver-assistance systems (ADAS) and telematics that constantly monitor driving patterns.
"Our cars talk to us these days, so pay attention," urges Dr. Tappen. "The car can provide you with some feedback and let you know that you might want to be more aware and have an evaluation done." This shifts the paradigm from reactive fear to empowered, data-informed self-care.
The FAU study is ongoing, with video data analysis forthcoming to provide even deeper behavioral insights. The ultimate vision is a fusion of automotive technology and healthcare, where consented, anonymized driving data could help clinicians spot early warning signs, much like routine blood work screens for other health conditions. For now, the message to seniors and their families is one of informed awareness: changes in driving are not just about safety on the road; they may be a critical signal about brain health, warranting a timely conversation with a doctor.
